- Family income less than or equal to 100% of the federal poverty level
- Income between 100% & 200%
- Over-income (up to 10% of families exceeding income guidelines)

- If Employed, previous year’s tax return, W2, letter from employer, current pay check stub
- If Self-Employed, copy of form 1099
- If Unemployed, copy of OWF, SSI, Social Security, Unemployment, Child Support, etc.
Proof of No Income Declaration
- Registration Office will provide more information
Proof of Work/School/Training Schedule
- Family must reside in Summit County
- Letter from Employer or Pay Stub
- School and Training Schedule
- Any other form of documentation that will provide information
Other Critical Information
- Up-to-date Shot Record
- Custody/Guardianship Papers, if applicable
- Insurance Card/Health Card/Child’s last four digits of Social
- Security number
- Doctor’s Name, Address and Phone Number
- Dentist’s Name, Address and Phone Number
